Ask Dr. K: What is the quickest way to get a beach ready body?

Ask Dr. K: How Do I Avoid The Tell-Tale Signs of a Facelift

I always stress that the most important part of cosmetic surgery, is choosing your doctor. If you don’t perform thorough research ask the right questions during a consultation, you may end up with something you didn’t ask for. Surgery scars are one of the most important aspects to getting something done, especially when that surgery is on your face. I spend most of my days in Beverly Hills and after traveling internationally, I see far too many unfortunate cases of women and men who have had facelift, but are left with distracting and obvious scarring. Sometimes these scars are seen around the ear, or are seen in the form of a raised hairline. No matter what the obvious scar may be, it is very unfortunate and speaks not of the person, but of the surgeon who performed the surgery. But unfortunately, it is the innocent patient who has to live with it.
When researching a doctor for a facelift, ask about their training and board certification and whether they have specialized training in performing facelifts. Also, be sure to closely inspect their work and ask to see images of facelifts they have performed in the past. Ask detailed questions about their procedure technique and how they avoid scarring.

Ask Dr. K: Is Your Surgeon Board Certified?

Is Your Surgeon Board Certified?
It may seem like an obvious question, but many patients often forget to consider whether or not their surgeon is board certified. It is imperative to know this before considering surgery and to know that not all surgeons are board certified. Keep in mind, if your surgeon is board certified, you want to make sure it is within the specialty for which you are seeing them. For example, an ENT doctor may be board certified, but that does mean he can perform plastic surgery. So, you want to know what the specialty or sub-specialty is and make sure the certification is for what you are looking for.

What Does It Mean To Be Board Certified, Is It Different From Being Licensed? 
Yes. Every physician MUST be licensed to practice medicine. Medical boards in the US and other jurisdictions issue these licenses and have the authority to investigate, discipline the practitioner and regulate the practice of medicine within their state or territory.
In addition to be licensed, some physicians may be board certified in their specialty. For example, I am board certified in both General Surgery and Plastic Surgery. These boards are very different from the state boards that issue licenses. Every recognized medical specialty and many sub-specialties have established boards to examine the qualifications of the practitioner. Each specialty board determines the appropriate qualifications with examinations, competency demonstrations and training, to name a few, which show an acceptable minimum level of knowledge. If acceptable, that practitioner will then be awarded a certification to show they meet the qualifications.
Is a Board Certified Surgeon Better Than One That Is Not Certified? 
In my opinion, yes. Not being certified does not necessarily mean the surgeon lacks the skills, but knowing that your surgeon is certified can give you peace of mind knowing that they have a board certification behind them. Board certification demonstrates that the surgeon has met the requirements of a particular specialty or for that particular certification. To be certified involves successful completion of a specific amount of training and passing an exam. Some surgeons who are not certified may be just as skilled, however there is something to be said if the surgeon is unable to or has not applied for board certification.

Ask Dr. K: Liposuction & Breast Reduction Surgery for Men?

Ask Dr. Kassabian: Men Getting Breast Reduction Surgery? 

I have been wanted to discuss this for some time and figured no better than now with Father’s Day just around the corner. This month I will discuss the topic of men receiving Breast Reduction Surgery. Well, it is more common that you might think.

Many men feel embarrassed or insecure by enlarged breasts, technically known as Gynecomastia. About 50 percent of all men have excess localized fat or excess glandular tissue in their breasts. According to the American Society of Plastic Surgeons, gynecomastia accounts for more than 65 percent of all male breast disorders. So we can see, enlarged breasts in men affects a significant portion of our population, so stigmas can be put to rest with the realization of these numbers and advances in Male Breast Reduction Surgery.

I would like to note that just because a male may have “larger than average” breasts, does not mean he has Gynecomastia. Some men and boys store fat on their chests, giving the appearance of breasts. This is called Pseudogynecomastia and may go away with diet and exercise. Although not as severe as Gynecomastia, one may seek further treatment after diet and exercise have proven futile in eradicating the fat pockets.

Although not a serious medical problem, Gynecomastia can cause physical pain and inflict emotional distress and can be seen in both men and young boys.  In fact, up to 65 percent of 14-year-old males have enlarged breasts, according to the American Academy of Family Physicians. 90 percent of these cases will see the excess breast tissue disappear within 3 years, largely due fluctuations in sex hormones during puberty. In older men, gynecomastia often results from the decreasing levels of testosterone, which occurs naturally during the aging process. Men also experience an increase in body fat with age and a resultant rise in estrogen, adding to the condition.

Now that we can see how common the occurrence of enlarged breasts in men is, gynecomastia or not, we can explore treatment options.

Liposuction and Laser Liposuction are the most viable solutions for such a common condition, both with minimal recovery time. Here, a small incision is made and a hollow tube (cannula), attached to a pump, is inserted and used to suction the excess fat out.

In more severe cases, an Excision procedure may be performed, in which the surgeon will remove the excess glandular tissue, fat and skin from the surrounding area around the areola and from the sides and bottom of the breast. Depending on their severity of each case, the incision size will vary, but with proper post-surgery care, scarring can be minimized significantly.

Male Breast Reduction Surgery is most often performed on an out-patient basis and is complete in a matter of hours with very little downtime following. Each case is different but one aspect remains constant, enlarged breasts in males is common, easily treatable and most importantly, nothing to be ashamed of.
