My patients often ask me what makes my breast lift techniques so unique from other plastic surgeons. There are several factors that separate my Breast Lift surgery from others.  Not only do I reposition the nipple and tighten the skin, both standard in breast lifting procedures, but my “invisible bra” technique re-structures and re-distributes your own breast tissue to create the perfect contour and shape.

In other words, the traditional procedure lifts only the skin, whereas my technique actually lifts the breast tissue providing support and contour and has a long-lasting effect. It’s like an invisible bra, the shape and volume of the breast is modified to give a perky and youthful silhouette.

These results are achieved even without the use of breast implants (if you have enough breast volume to start with) since the contouring effect gives the necessary upper pole fullness.

Ask Dr. K: What Is A Chin Augmentation?

A Chin Augmentation is a very common procedure in my practice with the aim of bringing balance and harmony to the proportions of the patient’s face. This is achieved by way of correcting a weak chin due to congenital deficiency, age-related bone absorption or facial trauma.

The procedure can vary in terms of process and magnitude depending on the severity and the desired result. It can be as simple as injecting a temporary filler to the area to  implanting a silicon chin implant, to even having a genioplasty, more commonly known as chin repositioning.

Reasons patients either request a chin implant or are advised to do so is to enhance the overall balance of the face. For instance, a weak chin make one’s nose appear larger and can even make the neck appear to have more fat. Adversely, a chin that is too large will steal focus from other features and will dominate the person’s appearance. Ideally, there should be a balanced relationship between the features and the structure of the face.
Although chin augmentation can be a routine procedure it is crucial, as with any surgery, that you do your research. Is your doctor board certified? Has your doctor performed this before and can you see before and after images? You can never be too informed when it comes to your health and safety so be sure to ask the tough questions and really make sure this something you want.

Ask Dr. K: What is the quickest way to get a beach ready body?

It’s the middle of summer and many of my patients are looking to me for quick solutions to trip up look better in their swimsuits.  There are several, easy ways to tighten up your body so you feel more comfortable in the less amount of clothing that summer weather permits.

My first suggestion is nutrition. Drink lots of water and incorporate as many raw fruits and vegetables as you can into your diet. Try to cut out dairy, sugar and of course processed foods. Not only will this help trim the fat, but will help your skin glow and you will feel better all around.

Ask Dr. K: What Are Micro-Cannulas and How Do They Work?

Many patients ask me about my techniques regarding injectables and fillers and how I am able to achieve such natural results with minimal injection points and a smooth and even texture. My technique for injecting is a large part of the success of my treatments but an important factor in these results is the use of a micro-cannula instead of the traditional needle.

A micro-cannula is a very thin, small and flexible hollow tube with a smooth round tip. There is no sharp end as you would see in a traditional needle. The benefit with this design is the minimal entry points that are required to achieve a result as opposed to the many punctures you would need with a traditional needle with less pain and a much less probability of bruising.

Some of the benefits of a Micro-Cannula over a needle include:

  • Reduced Pain
  • Less Bleeding
  • Less Bruising
  • Less Tissue Trauma & Swelling

The micro-cannula injection requires a single entry point for a large area instead of many with a traditional needle. The tip of cannula is smooth and rounded which allows the vessels to be simply pushed aside instead of being cut. This means, instead of pain, the patient will most likely experience little more than pressure. Whether injecting the under eyes or lips, you can be assured that bruising and swelling is minimal and you can continue with your everyday activities and attend events right away.

The process of injecting is simple. An entry point is made with a traditional needle that is, of course, slightly larger than the micro-cannula. A numbing agent, like Lidocaine may be used to ease an discomfort one might feel, but pain is very slight.

Ask Dr. K: PIP Implants and How They Affect You

French company Poly Implant Prosthese (PIP) came under fire in December of 2011 after revelations about their silicone implant products. It was learned that these implants actually contained industrial grade silicone instead of medical grade silicone and are more prone to rupture and leakage. The marketing, distribution and use of the PIP implants was halted in March 2010. It has been reported that the company was actually using a cheap type of silicone gel intended for making mattresses.
With regards to preventative measures, an expert committee was set up recently to examine the specific risks associated with PIP implants. It has been concluded that there was not enough evidence to recommend early removal. More than 300,000 PIP implants have been sold globally in 65 countries over the past 12 years. Europe was a major market but over half of the implants went to South America. The sale of these implants in the United States was blocked by the FDA in 2000 but U.S. partner, MediCor, Ltd continued to book sales of the implants until 2002. There are several documented cases revealing surgeons using and women receiving these implants even after the block. One New Mexico surgeon even said that 15 out of the 20 PIP devices he used eventually failed.
How do I know if I have PIP implants?
Contact your doctor who performed your surgery immediately and inquire about the type of implants you received.
How can I tell if there is a rupture in my PIP implants?
A rupture is a split that occurs in the implants casing and can be caused if:
-The implants shell gets weaker over time
-The implants is damaged during operation
-There is a flaw in the implant
-The breast is injured, such as a car accident, etc.
If you have any of the following symptoms, contact Dr. Kassabian immediately:
-Lumpiness of the breast
-Lumpiness or swelling in the area around the breast
-Change in shape of the breast
-Deflation of the breast
-Tenderness of the breats
-Swelling of the breast
-Pain or sensitivity

Ask Dr. Kassabian: Men Getting Breast Reduction Surgery? 

I have been wanted to discuss this for some time and figured no better than now with Father’s Day just around the corner. This month I will discuss the topic of men receiving Breast Reduction Surgery. Well, it is more common that you might think.

Many men feel embarrassed or insecure by enlarged breasts, technically known as Gynecomastia. About 50 percent of all men have excess localized fat or excess glandular tissue in their breasts. According to the American Society of Plastic Surgeons, gynecomastia accounts for more than 65 percent of all male breast disorders. So we can see, enlarged breasts in men affects a significant portion of our population, so stigmas can be put to rest with the realization of these numbers and advances in Male Breast Reduction Surgery.

I would like to note that just because a male may have “larger than average” breasts, does not mean he has Gynecomastia. Some men and boys store fat on their chests, giving the appearance of breasts. This is called Pseudogynecomastia and may go away with diet and exercise. Although not as severe as Gynecomastia, one may seek further treatment after diet and exercise have proven futile in eradicating the fat pockets.

Although not a serious medical problem, Gynecomastia can cause physical pain and inflict emotional distress and can be seen in both men and young boys.  In fact, up to 65 percent of 14-year-old males have enlarged breasts, according to the American Academy of Family Physicians. 90 percent of these cases will see the excess breast tissue disappear within 3 years, largely due fluctuations in sex hormones during puberty. In older men, gynecomastia often results from the decreasing levels of testosterone, which occurs naturally during the aging process. Men also experience an increase in body fat with age and a resultant rise in estrogen, adding to the condition.

Now that we can see how common the occurrence of enlarged breasts in men is, gynecomastia or not, we can explore treatment options.

Liposuction and Laser Liposuction are the most viable solutions for such a common condition, both with minimal recovery time. Here, a small incision is made and a hollow tube (cannula), attached to a pump, is inserted and used to suction the excess fat out.

In more severe cases, an Excision procedure may be performed, in which the surgeon will remove the excess glandular tissue, fat and skin from the surrounding area around the areola and from the sides and bottom of the breast. Depending on their severity of each case, the incision size will vary, but with proper post-surgery care, scarring can be minimized significantly.

Male Breast Reduction Surgery is most often performed on an out-patient basis and is complete in a matter of hours with very little downtime following. Each case is different but one aspect remains constant, enlarged breasts in males is common, easily treatable and most importantly, nothing to be ashamed of.

Ask Dr. K: The Truths and Myths of Anesthesia

Dear Patients and Friends, 

One of the most important topics I discuss during a consultation is anesthesia. Many misunderstandings exist and I want clear up the truths and myths about the use of anesthesia.

What is anesthesia?

Anesthesia controls pain during surgery. To keep you comfortable and relaxed, medicines are used. Along with anesthesia, close monitoring is used to control breathing, blood pressure, blood flow, and heart rate and rhythm.

What are the different types of anesthesia?

Local anesthesia numbs a small part of the body. You get an injection of the anesthetic directly into the surgical area to block pain. Local anesthesia is used only for minor procedures. Regional anesthesia blocks pain to a larger part of your body. Anesthetic is injected around major nerves or the spinal cord. Major types of regional anesthesia include:

  • Peripheral nerve blocks: An injection of anesthetic near a specific nerve or group of nerves. It blocks pain in the part of the body supplied by the nerve. Nerve blocks are most often used for procedures on the hands, arms, feet, legs or face.
  • Epidural and spinal anesthesia: An injection of anesthetic near the spinal cord and the nerves that connect to it. It blocks pain for an entire region of the body, such as the belly, hips or legs.
  • General anesthesia: Affects the brain as well as the entire body. You may receive it through a vein (intravenously, or IV), or you may breathe it in. With general anesthesia, you do not feel pain during the surgery. It causes you to forget the surgery and the time right after it.

There are several benefits to have anesthesia with regards to cosmetic surgery:

o   With facial or body surgery, anesthesia relaxes, relieves pain and keeps the patient comfortable and still during surgery. Any movements could potentially harm the patient and complicate the surgery, such as contamination of the sterile field or nerve injury.

o   With liposuction, fat is removed more effectively using general anesthesia rather than local technique. Less of the local numbing agent (tumescent) is required resulting in more fat removed. Under general anesthesia, the end result is a more precise contour, and a pleased patient.

o   I perform very few procedures with a paralyzing agent and when I do, it is a very small amount. The paralyzing agent is given for a short duration of the surgery when muscle relaxation is important, such as breast augmentation or tummy tuck.

What determines the type of anesthesia used?

The type of anesthesia used depends on several things:

  • The reason for your surgery and the type of surgery.
  • Your past and current health.
  • The doctor will consider other surgeries you have had and any health problems you may have such as heart disease, lung disease, or diabetes.
  • The results of tests, such as blood tests or an electrocardiogram (EKG).
